Appointments
The Reston office is open from 8 a.m. until 4 p.m. Monday through Friday, excluding major holidays and emergencies. Physician appointments (at Reston and/or Manassas locations) are by appointment only.
Please print the registration and medical history form from the web site. Bring your completed forms with you on the day of your appointment.

In addition to the registration form and history form, please remember to bring the following to your appointment:

X-ray, CT scan and or MRI films/disc pertaining to the reason for your visit. |
Failure to bring this information may necessitate rescheduling your appointment.
Please note it is the patient’s responsibility to know the requirements for his or her insurance plan. Contact your primary care physician if a referral is required to see a specialist. If you do not bring a referral, your appointment may be rescheduled. It is the patient’s responsibility to know the number of visits allowed and the expiration date of your referral. We cannot provide medical services to you without a valid referral.
We like to advise all patients that due to the nature of our specialty, when emergencies occur, we may have to reschedule office hours, and/or surgery to accommodate emergency surgery. Please contact our office to confirm your appointment prior to the appointment time. Previous
